Ordering Forms

As an Apostille Agent, you will need to communicate with the Office of the Secretary of State on a frequent basis.  We have provided you with a link to each Secretary of State.  It is important that you use these forms as some of the offices will reject a job without the proper form.  Often these forms will contain the proper amount you need to send for expedited service and, most importantly, whether they accept Federal Express, UPS or DHL.  For example, the Secretary of State’s Office in Pennsylvania does not accept Federal Express and if you send something, it will go the Federal Express graveyard and your job will more than likely disappear.
